  • Taking a Leave of Absence: Your Simple Guide

    Here's a simplified step-by-step guide for you on reporting different types of leave of absence:
    1. Tell Your Manager: Let your manager know you need time off. Inform them if it's FMLA, Non-FMLA, Personal/Unpaid Leave, or Disability Leave.
    2. Manager Notifies Benefits: Your manager will inform the Benefits Department about your leave request.
    3. Benefits Department Contacts You: Expect them to reach out to you for more details about your leave, such as its duration and reason.
    4. Register on FMLA Platform: You'll need to sign up on the FMLA Manager Employee Center to manage your leave details.
    5. Benefits Process Your Request: The Benefits Department will handle your request and keep you updated through the FMLA Manager Employee Center.
    6. Receive Rights Info: You'll receive a notice detailing your rights and responsibilities under the Family and Medical Leave Act.
    7. Leave Type Confirmation: They will send you a notice confirming the type of leave and its duration.
    8. Provide Certificates if Needed: If necessary, you have 15 days to provide any required documents, such as medical notes.
    9. Leave Starts: Your leave will start as indicated in the notice.

    Concurrent Disability and Workers’ Compensation Leave: If applicable, remember that these types of leave will run alongside your FMLA leave.

  • Maintaining Your Benefits During Your Leave

    1. Premium Responsibility: While on leave, remember you're responsible for the same benefits premiums as when you were actively working.
    2. Invoice via Email: You'll receive an email invoice for any premiums due during your leave.
    3. Billing Frequency: Premiums are billed twice a month, with due dates on the 10th and 25th.
    4. Payment Importance: If payments aren't made during your leave, coverage could be cancelled retroactively from the last paid-through date.
    5. Direct Billing Period: You have a maximum of 12 weeks for direct billing of premiums.
    6. COBRA Option: After 12 weeks, COBRA coverage will be available to continue benefits.
    7. Separate from Employment: Remember, your benefit coverage is independent of your employment status. Cancelling benefits due to non-payment won't affect your employment.

  • Ready to Return to Work?

    1. Exhaustion Notification: The Benefits Department will send you a letter before your leave ends, with your expected return to work date. If your return date changes or you won't be returning, reach out to us right away.
    2. Extension Request: If your return date changes, request an extension through the FMLA Manager Employee Center. For extensions beyond 12 weeks, we'll assess them case by case. Any granted extension will be treated as Personal/Unpaid leave.
    3. Full Duty Release: If you're fully cleared by medical documentation, return to your regular position on your next scheduled shift. Share the medical paperwork with the Benefits Department during business hours and before your next shift.
    4. Light Duty Release: For light duty restrictions, give the documentation to the Benefits Department ASAP. We'll work with your manager to decide if accommodations are possible. We'll let you know the outcome. If you can return, do so on your next scheduled shift. Not showing up may affect your attendance record.
